Application Description
The “Patient Handover” is the perfect tool for concise, accurate and logged patient record keeping and clinical handovers.
The APP has not only been used for medical, surgical and intensive care patients in the hospital setting but it has found popularity outside the hospital during clinical trials, research and in the “Pre-Hospital” environment.
The simplicity of “Creating” the hospital, clinic or theatre layout makes this one of the most user friendly APPs of its type.
Once the areas or patient groups are created they can be populated with beds or subjects. If patients are move around the physical areas within a hospital they can simply and quickly be moved around within the APP.
For each bed space, clinical subject or area created a patient’s demographics can be entered. This information can subsequently be amended and forms the basis of future searches.
The first four locations to add information about a patient are headed Background, Past Medical History, Drug History and Injuries. Each of these allows free text to be recorded.
This is followed by a system based review under the headings; Central Nervous System, Cardio Vascular System, Respiratory System, Gastro Intestinal System, Renal / Metabolic and Host Defence. Each of these categories starts with space for free text but then can be expanded under a series of drop down menus covering the salient clinical findings relevant to most major medical specialties.
As the patient’s notes progress the notes can be amended. Each time the APP is saved or an “Auto Save” time is set, these changes are highlighted by appearing as white text. Each time events are saved they populate the summary.
There are further spaces to record Jobs, Nursing Issues and Patient Status (resuscitation, escalation of care…). As jobs are completed they are “crossed off” but a record of this is kept in the summary.
Handing over or passing on the records couldn’t be simpler or safer. Patients’ records can be blue toothed to other approved devices. The template of the hospital layout can also be imported or exported from other devices.
A summary of a patient’s clinical findings or events is populated throughout their stay. The summary remains succinct as only new events or changes are recorded for the summary. This summary can be exported and used as a patient record or discharge summary in its own right.
