Based on the LACE index1 and the Charlson comorbidity2 index, this app assesses whether a patient is at risk for being readmitted within 30 days upon discharge. The Readmission Risk Calculator gives a final score from 0 to 19. Patients with a score of 10 to 11 or greater are at higher risk for being readmitted within 30 days. The Readmission Risk Calculator is available as a spreadsheet and a papertool. It can be used within the context of a nursing care plan that identifies patients at risk for readmission.
