Medication-related adverse events due to inadequate reconciliation are more likely to
occur in hospitalized patients with certain identifiable risk factors. The REMS App is a tool that mediates
prospective and proactive identification of patients at risk for medication-related adverse events
and is designed to effectively mitigate that risk through the prompting of appropriate actions.

As an alert system the app will help maintain vigilance toward these patient safety issues and assist in prioritising the assignment of additional resources on high-risk patients.

REMS is an application designed around the key concepts detailed in FDA Risk Evaluation and Mitigation
Strategy (REMS). The application is provided with predefined risk criteria and associated mitigation strategies as specified by Clinical Pharmacist Jason Asleson (Pharm.D) of The University of Utah.
Users can easily record and review the risk assessment for their patients as part of their daily routine,
appropriate mitigating strategies are in effect prompted to the user. The implementation of mitigation strategies can also be recorded to assist in monitoring and assessment of the current state of care for each patient. Additionally users can enter their own risk and mitigation criteria and modify the predefined criteria as required.

A simple scoring system is used to help highlight patients who are currently at greatest risk and where there are opportunities for mitigating actions to be put in place.

Patient data is limited to a subset of noncritical details and it is never passed through electronic communication, it resides solely in the secure application sandbox on your Apple device.
