With Servicebook you have all infos on one place: Servicebook uniquely combines address book and appointment management.

Dates to the doctor, in the garage or in the fitness center: With Servicebook you can create a business card for each of your service providers based on an address book entry: Simply pull down the main view to add a new card.

Turn a card to view or add any information like office hours at the doctor, insurance policy number, motor vehicle records, etc.

A new appointment at your dentist: For each appointment, you can optional add a title and a note. The card shows you all the appointments from the past. And you can select a calendar of the iOS calendar app or a list of the iOS reminder app for each card and every new date will also be saved in the calendar or reminder.

If you want to close a card on the iPhone, tend your device.

If an appointment will come soon, the corresponding card opens automatically: Each card can have an arbitrary number of alerts.

You want contact your doctor: open the doctor card, tap on the title and the address book entry will be shown. Also, you can add a second address book link on each card.

And whenever there is something to delete, left wipe on the corresponding row.

(Info for version 1.0: We´ve learned that some users tap on the plus button to select an address from address book. The plus button is for adding a new address, not for selecting one: If you want select an existing address from address book, tap left from the plus button on "Address". The plus button will be removed in the first update).
